New clients will need to complete the "Client Agreement & Release Form" as well as the "Your Whole-ness" form and send them to Kate via fax, e-mail, drop-off, or snail mail two days before your initial appointment. This allows her to develope an exploratory plan right from the start.
This is your first step in commitment. Please print this out, fill in the blank spaces, sign and initial where neccessary and drop off or mail to Kate before your initial session.
Please fill out the questions on this form as well as you can. If there is a particular question you don't understand or are hesitant to fill out we may discuss it during your meeting.
